Q. When can I make changes to my benefit elections?

A. You may make changes during annual enrollment or within 31 days of when you experience a qualified status change. In the case of a qualified status change, the effective date of the coverage will be the date the status change occurred.


Q. What do I have to do if my banking information has changed for auto withdrawal?

A. Complete a new Automatic Deduction Form and return to the Benefits Office for processing.

Q. What do I do if I am currently in the Blue Cross Blue Shield HMO plan and will be moving out of state?

A. You need to complete the Coverage Change Request Form and the Retiree Medical Enrollment Form. Both forms must be submitted to the Benefits Office within 30 days of the change.

If the change is not received within 30 days of the change date you will need to wait until Annual Enrollment to change plans. Lastly, you also must complete the address change form and submit the address change.

Q. I am enrolled in the dental plan through COBRA, what do I need to do if I am interested in conversion information after the 18 months of coverage?

A. If you are interested in converting your dental coverage please contact Delta Dental at 1-800-323-1743 within 90 days of the date your COBRA coverage ends and request a dental conversion packet be mailed to your home.
